Medical Necessity
Medical necessity is the standard insurers use to decide whether a service is appropriate, effective, and needed to diagnose or treat your condition. A denial for lack of medical necessity means the plan is not convinced the care meets its criteria, not necessarily that the care is wrong. These are among the most appealable denials because they turn on clinical evidence you and your provider can supply.
