Letter of Medical Necessity

A letter of medical necessity is a statement, usually written by your treating provider, that explains why a specific service or medication is appropriate and necessary for your condition. It ties your diagnosis and history to the requested treatment and often cites supporting evidence. A strong letter is frequently the deciding factor in overturning a medical-necessity denial.
